1. General Declaration and Assumption of Risk

I hereby declare, in full awareness of my responsibility, that my health condition meets the physical and mental requirements necessary for participation in the escape room game, and I am not aware of any medical problem that could lead to harm to my health during or after the game. I declare that I am participating in the game at my own risk, and I am obligated to inform the staff in advance if, due to a known medical condition (e.g., epilepsy, panic disorder, PTSD, allergies, asthma, etc.), the types of effects present in the game may pose a risk to me.

2. Declaration Regarding Specific Medical Conditions

I declare that I do not currently have any of the following medical conditions:

• Cardiovascular diseases, such as high blood pressure, arrhythmia, or a previous heart attack;

• Pregnancy – I acknowledge that participation is not recommended during pregnancy due to enclosed spaces and potential stress;

• Epilepsy or a predisposition to seizures that can be triggered by strong light, sound, or visual stimuli;

• Severe psychological or mental disorders, such as panic disorder, schizophrenia, or severe depression;

• Recent surgery or injury, especially in the musculoskeletal area (bone, muscle, joint);

• Chronic respiratory diseases, including asthma or other breathing difficulties;

• Osteoporosis or an increased risk of bone fractures;

• Medication that reduces alertness, slows reaction time, or affects judgment (e.g., sedatives, strong painkillers);

• Vision or hearing impairment that hinders the perception of dangers or the safe use of game elements;

• Chronic musculoskeletal problems – such as lower back, back, or knee pain, which may be aggravated during the game.

3. Acknowledgment of Liability and Final Confirmation

I acknowledge that if I conceal any information related to my health condition, and as a result, health damage, injury, or illness occurs during the game, it is solely my responsibility. The service provider (operator) shall not be held liable for the consequences arising from such incidents, with the exception of the mandatory liability prescribed by law.

By signing, I certify that I have completed the above declaration truthfully and to the best of my knowledge, and I fully understand the risks and conditions of participating in the game.

I have familiarized myself with the provisions of the House Rules and Game Regulations, and I accept the health warnings contained therein.
